Foundayo™ (orforglipron) (17.2mg tablet (max))
Foundayo™ (orforglipron) is the first per os non-peptide (small-molecule) GLP-1 receptor agonist, approved by the FDA in April 2026 (Eli Lilly) for weight management in adults with obesity or overweight with comorbidities. Unlike Rybelsus®, it does not require fasting or timing restrictions for absorption.

How It Works
Orforglipron is a **non-peptide, small-molecule** partial agonist of the GLP-1 receptor. Unlike peptide GLP-1 RAs (semaglutide, liraglutide), its non-peptide structure allows for stable per os bioavailability without the need for absorption enhancers (like Rybelsus's SNAC). It activates GLP-1 receptors in pancreatic beta-cells (increasing glucose-dependent insulin secretion), the hypothalamus (reducing appetite), and the GI tract (slowing gastric emptying).
Quick Start
- Form: Film-coated tablets — 0.8 / 2.5 / 5.5 / 9 / 14.5 / 17.2 mg.
- Dosing: Start at 0.8 mg → titrate every ≥30 days (max 17.2 mg).
- Timing: Anytime during the day, with or without food.
- Swallow whole — do not break, dissolve, or chew.
Official Dosing Titration — Foundayo™ (FDA USPI 04/2026)
Route: Oral (per os) | Frequency: Once daily
| Week | Dose | Amount |
|---|---|---|
| Wks 1–4 | 0.8 mg (800 mcg) | 1 tablet 0.8mg |
| Wks 5–8 | 2.5 mg (2,500 mcg) | 1 tablet 2.5mg |
| Wks 9–12 | 5.5 mg (5,500 mcg) | 1 tablet 5.5mg |
| Wks 13–16 | 9 mg (9,000 mcg) | 1 tablet 9mg |
| Wks 17–20 | 14.5 mg (14,500 mcg) | 1 tablet 14.5mg |
| Wks 21+ | 17.2 mg (17,200 mcg) — max | 1 tablet 17.2mg |
Take once daily, at any time, with or without food. Titration occurs only after ≥30 days at the current dose, based on response and tolerability. Maximum dose: 17.2 mg/day. In co-administration with a strong CYP3A4 inhibitor: max 9 mg/day.

Protocol Overview
- Orforglipron was approved by the FDA on April 1, 2026, as the first non-peptide, small-molecule oral GLP-1 receptor agonist.
- Unlike Rybelsus® (oral semaglutide), which requires fasting and a 30+ minute waiting period, Foundayo can be taken anytime with or without food — improving adherence.
- In ATTAIN-1 trials (obesity without T2D, 72 weeks), weight loss of ~12–14% was achieved at the maximum dose. In ACHIEVE-1 (T2D): HbA1c reduction of 1.3–1.6% and weight loss of 7–8%.
- The EMA assessment process is ongoing — no EMA approval yet.
Dosing Protocol
- Initiate with 0.8 mg once daily for 4 weeks.
- After ≥30 days, increase to 2.5 mg/day.
- After ≥30 days, increase to 5.5 mg/day.
- Optional titration to 9 mg → 14.5 mg → 17.2 mg, with ≥30 days at each level.
- Missed dose: if >12 hours have passed, skip and resume normal dosing the next day.
- Break between cycles: A 2–4 week break is recommended before repeating the cycle.
Washout & Cycle Restart
When can I restart?
After completing the cycle, a 2–4 week break is recommended before starting a new cycle. This allows the body to restore normal function and prevents potential downregulation (tachyphylaxis).
Storage Instructions
- Store at room temperature (20–25°C, with excursions permitted 15–30°C).
- Keep in the original blister pack, protected from moisture.
- Keep away from direct sunlight and out of reach of children.
Important Notes
- ⚫ Boxed Warning (FDA): Possible risk of thyroid C-cell tumors (class effect for GLP-1 RAs in rodents).
- ⚠️ Contraindications: Personal or family history of medullary thyroid carcinoma (MTC) or MEN 2 syndrome.
- Severe gastrointestinal reactions — not for use in severe gastroparesis.
- Risk of acute pancreatitis — discontinue if suspected.
- Risk of acute kidney injury from dehydration (vomiting/diarrhea).
- Oral contraceptives: use alternative or additional barrier methods for 30 days after initiation and after each dose escalation.
- Severe hepatic impairment: not recommended.
- Pregnancy/lactation: contraindicated.
- Educational information only — prescribing and titration should be managed exclusively by a licensed healthcare provider.
