Oral Semaglutide (Rybelsus®) (14mg tablet)
Oral Semaglutide (Rybelsus®) is the first FDA/EMA-approved per os (oral) GLP-1 agonist for type 2 diabetes. It uses the enhancer SNAC (sodium N-[8-(2-hydroxybenzoyl)amino]caprylate) to increase gastric absorption. The PIONEER (1–10) clinical trials have documented its efficacy in glycemic control and body weight reduction.

How It Works
Semaglutide is a GLP-1 receptor agonist (94% homology with native GLP-1). It activates GLP-1 receptors in pancreatic β-cells (increasing insulin, decreasing glucagon), the hypothalamus (reducing appetite), and the GI tract (slowing gastric emptying). The SNAC enhancer locally increases pH and allows for paracellular absorption.
Quick Start
- Form: Tablet 3mg / 7mg / 14mg — no reconstitution required.
- Dosing: Start 3mg → titrate to 7mg → 14mg every 30 days.
- Administration Time: In the morning on an empty stomach, ≥30 minutes before food/drink/medication.
- Swallow whole with ≤120 mL of water — do not split, crush, or chew.
Typical Dosing Titration — Rybelsus® (Per Os)
Route: By mouth (per os) | Frequency: Once daily
| Week | Dose | Amount |
|---|---|---|
| Day 1–30 | 3 mg (3,000 mcg) | 1 tablet 3mg |
| Day 31–60 | 7 mg (7,000 mcg) | 1 tablet 7mg |
| Day 61+ | 14 mg (14,000 mcg) | 1 tablet 14mg |
Take once daily, preferably in the morning, on an empty stomach with ≤120 mL of water. Wait at least 30 minutes before eating, drinking, or taking other medications. Food significantly reduces bioavailability.

Protocol Overview
- Per os Semaglutide (Rybelsus®) was approved by the FDA (2019) and EMA (2020) for type 2 diabetes. It is the first GLP-1 receptor agonist in tablet form.
- The SNAC enhancer (300mg per tablet) creates a local pH increase in the gastric mucosa, allowing for paracellular absorption of the peptide.
- Bioavailability is ~1% — the per os dose (14mg) is significantly higher than the injectable dose (e.g., up to 2.4mg for Wegovy) due to low absorption.
Dosing Protocol
- Start with 3mg daily for 30 days (titration).
- Increase to 7mg daily for the next 30 days.
- If needed, increase to the maximum dose of 14mg daily.
- Take on an empty stomach, 30+ minutes before food/liquids/medications.
- Break between cycles: A 2–4 week break is recommended before repeating the cycle.
Washout & Cycle Restart
When can I restart?
After completing the cycle, a 2–4 week break is recommended before starting a new cycle. This allows the body to restore normal function and prevents potential downregulation (tachyphylaxis).
Storage Instructions
- Store at room temperature (15–30 °C).
- Keep in the original blister pack until use.
- Protect from moisture and direct sunlight.
Important Notes
- ⚠️ Approved medication (FDA/EMA) — use only under medical supervision.
- Contraindicated in MEN2 syndrome and a personal or family history of medullary thyroid carcinoma.
- Risk of pancreatitis — report severe abdominal pain immediately.
- Food, liquids, or medications taken within 30 minutes drastically reduce absorption.
Potential Benefits & Side Effects
Potential Benefits:
- HbA1c reduction of 1.0–1.4% (PIONEER trials)
- Body weight loss of 3–5 kg (14mg dose)
- Cardiovascular risk reduction (SOUL trial, pending results)
- Per os administration — avoids injections
- FDA/EMA approved — strong safety documentation
Potential Side Effects:
- Nausea (most common, usually transient)
- Diarrhea
- Decreased appetite
- Abdominal pain
- Pancreatitis (rare)
- Gallstones
⚠️ Serious Warnings & Long-Term Risks
The following warnings are based on published literature, FDA labels, and post-marketing surveillance. They do not constitute medical advice. Consult a healthcare professional.
Thyroid C-cell tumors (medullary thyroid carcinoma)
In rodents, semaglutide caused dose-dependent thyroid C-cell tumors (medullary carcinoma). Contraindicated in patients with personal/family history of MTC or MEN 2.
