Mazdutide (5mg)
Mazdutide (LY3305677) is a dual GLP-1/glucagon agonist designed for weight loss and glycemic control. In Phase 2/3 clinical trials, it demonstrated up to 14% weight loss over 24 weeks. Weekly subcutaneous administration with gradual titration.

How It Works
Mazdutide activates both GLP-1 and glucagon receptors. GLP-1 agonism reduces appetite, slows gastric emptying, and enhances insulin secretion. Glucagon agonism increases energy expenditure through thermogenesis and hepatic lipolysis, providing an additional advantage in weight reduction.
Quick Start
- Reconstitution: Add 2.0 mL bacteriostatic water → 2.5 mg/mL concentration.
- Weekly range: 3–9 mg once weekly.
- Storage: Lyophilized: −20 °C; reconstituted: 2–8 °C, within 28 days.
Gradual Titration (2 mL = 2.5 mg/mL)
Route: Subcutaneous | Frequency: Once weekly
| Week | Dose | Units (per injection) |
|---|---|---|
| Weeks 1–4 | 3 mg | 2 × 60 Units (0.60 mL) |
| Weeks 5–8 | 4.5 mg | 2 × 90 Units (0.90 mL) |
| Weeks 9–12 | 6 mg | 3 × 80 Units (0.80 mL) |
| Weeks 13+ | 9 mg | 4 × 90 Units (0.90 mL) |
Studies report administration once weekly subcutaneously. Increase every 4 weeks if well-tolerated. The 5 mg vial is suitable for titration.

Reconstitution Steps
- Withdraw 2.0 mL of bacteriostatic water.
- Inject slowly down the side of the vial; avoid foaming.
- Gently swirl until dissolved.
- Note the date; refrigerate at 2–8 °C.
Protocol Overview
- Objective: Weight loss and glycemic control via dual GLP-1/glucagon agonism.
- Schedule: Weekly injections for 12–24+ weeks.
Dosing Protocol
- Initiation: 3 mg/wk × 4 weeks.
- Escalation: 4.5 mg (Wk 5–8), 6 mg (Wk 9–12).
- Maintenance: 6–9 mg/wk.
- Break between cycles: After the cycle, a washout period of 4–8 weeks is recommended. Mazdutide (a dual GLP-1/glucagon agonist) has a long half-life — the break allows for metabolic restoration.
