FOXO4-DRI (10mg)
FOXO4-DRI dosing protocols focus on this innovative senolytic peptide (also known as Proxofim) designed to selectively target and eliminate senescent "zombie" cells. As a D-retro-inverso analog of a segment of the FOXO4 protein, it works by disrupting the FOXO4-p53 interaction in senescent cells, freeing p53 to trigger apoptosis and clearance of dysfunctional cells. This protocol outlines an intermittent subcutaneous approach.

How It Works
FOXO4-DRI is a cell-penetrating antagonist peptide that targets the FOXO4-p53 interaction in senescent cells. In senescent cells, FOXO4 abnormally accumulates in the nucleus and sequesters p53, preventing apoptosis. FOXO4-DRI was designed with D-amino acids (D-retro-inverso configuration) for resistance to degradation while mimicking the FOXO4 binding region. By displacing endogenous FOXO4 from p53, the peptide frees p53 to relocate to the mitochondria and trigger apoptosis specifically in senescent cells, while leaving healthy cells unharmed.
Quick Start
- Reconstitution: Add 3.0 mL of bacteriostatic water → 3.33 mg/mL concentration.
- Typical dose range: 300–500 mcg once daily during dosing weeks.
- Storage: Lyophilized: freezer at −20 °C; post-reconstitution, refrigerator at 2–8 °C; avoid freeze-thaw cycles.
Typical / Intermittent Pulse Approach (3 mL = 3.33 mg/mL)
Route: Subcutaneous | Frequency: Once daily (during dosing weeks)
| Week | Dose | Units (per injection) |
|---|---|---|
| Week 1 (Days 1–7) | 300 mcg | 9 Units (0.09 mL) |
| Week 2 (Days 8–14) | Off | No injection |
| Week 3 (Days 15–21) | 400 mcg | 12 Units (0.12 mL) |
| Week 4 (Days 22–28) | Off | No injection |
Studies report administration once daily subcutaneously during dosing weeks. Senolytics like FOXO4-DRI are more effective with intermittent "hit-and-run" dosing than with continuous administration.

Reconstitution Steps
- Withdraw 3.0 mL of bacteriostatic water.
- Inject slowly down the side wall; avoid foaming or vigorous agitation.
- Gently swirl/rotate until dissolved (do not shake).
- Note the date and refrigerate at 2–8 °C, protecting from light.
Protocol Overview
- Goal: Selective elimination of senescent cells to support tissue regeneration and restore healthy cellular function.
- Schedule: 1 week on, 1 week off cycles; 1–3 cycles total (2–6 weeks).
- Storage: Lyophilized frozen; reconstituted refrigerated; avoid freeze-thaw cycles.
Dosing Protocol
- Initiation: 300 mcg daily during the first dosing week.
- Target: 400–500 mcg daily in subsequent cycles if well tolerated.
- Frequency: Once daily (subcutaneously) only during dosing weeks.
- Cycle Structure: 7 days on → 7 days off; repeat 1–3 times.
- Break between cycles: After the cycle, a washout period of 8–12 weeks is recommended. Senolytics require a longer pause for senescent cell clearance and tissue remodeling.
Washout & Cycle Restart
When can I restart?
After the cycle, a washout period of 8–12 weeks is recommended. Senolytics require a longer pause for senescent cell clearance and tissue remodeling.
Minimum wait period: 8 weeks
Storage Instructions
- Lyophilized: Store at or below −20 °C; −80 °C is ideal for long-term.
- Reconstituted: Refrigerate at 2–8 °C; use within 28 days with bacteriostatic water.
- Allow vials to reach room temperature before opening.
- Protect from light; keep cap secure.
Important Notes
- Discard reconstituted solution after 28 days regardless of remaining volume.
Potential Benefits & Side Effects
Potential Benefits:
- Supports selective clearance of senescent cells while leaving healthy cells unharmed.
- Preclinical studies showed improved physical fitness, coat density, and organ function in aged mice.
- Research showed restoration of testosterone levels in aged male mice by clearing senescent Leydig cells.
- Reduced fibrotic tissue damage in models of pulmonary fibrosis.
- Animal studies reported no apparent adverse effects.
Potential Side Effects:
- Mild injection site reactions (redness) may occur.
- Note: No human clinical trials have been completed (2025); all data is preclinical.
⚠️ Serious Warnings & Long-Term Risks
The following warnings are based on published literature, FDA labels, and post-marketing surveillance. They do not constitute medical advice. Consult a healthcare professional.
Senolytic: potential tissue damage
As a senolytic, FOXO4-DRI induces apoptosis in senescent cells. Theoretical risk of damage to tissues relying on senescent cells (e.g., liver, endothelium). Preclinical data only (mice).
