Adipotide (5mg)
Adipotide (FTPP) is a synthetic peptide mimetic that targets prohibitin-1 in the vasculature of white adipose tissue, delivering a pro-apoptotic sequence for targeted fat mass reduction. Clinical development was halted after Phase I due to nephrotoxicity concerns. 5mg vial — reconstitute with 2 mL of bacteriostatic water for 2.5 mg/mL.

How It Works
Adipotide is a peptide mimetic designed to selectively target blood vessels within white adipose tissue, by binding to prohibitin-1 on the vascular endothelium. Upon binding, it delivers a pro-apoptotic sequence (D[KLAKLAK]₂) that triggers vessel destruction via an apoptotic mechanism, cutting off blood supply to fat cells. Preclinical studies in obese rodents showed ~30% weight reduction, while obese primates had ~11% weight loss over 4 weeks.
Quick Start
- Reconstitution: 2.0 mL bacteriostatic water → 2.5 mg/mL.
- Starting Dose: 250 mcg daily; gradually increase by 250 mcg/2 weeks.
- Easy Measurement: At 2.5 mg/mL, 1 unit = 25 mcg on a U-100 syringe.
- Storage: Lyophilized: −20 °C; reconstituted: 2–8 °C.
Gradual Approach (2 mL = 2.5 mg/mL)
Route: Subcutaneously | Frequency: Once daily
| Week | Dose | Units (per injection) |
|---|---|---|
| Weeks 1–2 | 250 mcg | 10 Units (0.10 mL) |
| Weeks 3–4 | 500 mcg | 20 Units (0.20 mL) |
| Weeks 5–6 | 750 mcg | 30 Units (0.30 mL) |
| Weeks 7–8 | 1000 mcg (1.0 mg) | 40 Units (0.40 mL) |
Inject once daily subcutaneously. Conservative start and slow titration to minimize potential renal side effects.
How Many Vials Will You Need?
8 Week Cycle (250→1000mcg/day)
7 vials
14×0.25 + 14×0.5 + 14×0.75 + 14×1.0 = 35mg
Total: 35mg (8 weeks)
12 Week Cycle (1000mcg/day maintenance)
13 vials
35mg + 28×1.0 = 63mg
Total: 63mg (12 weeks)
5mg vial ≈ 7 doses of 750 mcg or 5 doses of 1 mg.

Reconstitution Steps
- Draw 2.0 mL bacteriostatic water into a sterile syringe.
- Inject slowly down the side of the vial—avoid frothing.
- Gently swirl until fully dissolved—do not shake.
- Label with date and refrigerate at 2–8 °C, protected from light.

Protocol Overview
- Goal: Targeted fat mass reduction via vascular targeting.
- Schedule: Daily subcutaneous injections × 4–8 weeks.
- Storage: Lyophilized −20 °C; reconstituted refrigerated.
Dosing Protocol
- Initiate: 250 mcg daily Wks 1–2; increase by 250 mcg/2 weeks.
- Target: Up to 1000 mcg (1.0 mg) daily by Wks 7–8 if tolerated.
- Frequency: 1×/day subcutaneously.
- Duration: 4–8 weeks (extension with extreme caution).
- Timing: Consistent time; rotate injection sites.
- Break between cycles: 8-week break before repeating due to safety and nephrotoxicity concerns.
Washout & Cycle Restart
When can I restart?
8-week break before repeating due to safety and nephrotoxicity concerns.
Minimum wait period: 8 weeks
Storage Instructions
- Lyophilized: −20 °C, dry, dark.
- Reconstituted: 2–8 °C — avoid freeze-thaw cycles.
- Allow vials to reach room temperature before opening.
Important Notes
- Use new sterile syringes; discard in sharps container.
- Rotate injection sites (abdomen, thighs, arms).
- Inject slowly; wait a few seconds before withdrawing needle.
- ⚠️ Critical: Clinical development halted due to reversible nephrotoxicity at high doses. For educational purposes only.
- Renal function monitoring extremely important.
Potential Benefits & Side Effects
Potential Benefits:
- Targeted fat mass reduction (up to 30% in rodents, ~11% in primates).
- Mechanism specifically targeting adipose tissue vasculature.
- Data primarily from preclinical studies.
Potential Side Effects:
- Clinical development halted due to safety concerns.
- Dose-dependent reversible nephrotoxicity in preclinical studies.
- Mild local injection site reactions (redness, swelling).
- Long-term human safety profile: unknown.
⚠️ Serious Warnings & Long-Term Risks
The following warnings are based on published literature, FDA labels, and post-marketing surveillance. They do not constitute medical advice. Consult a healthcare professional.
Adipocyte destruction — potential local risks
Adipotide (FTPP) targets adipose tissue vasculature, causing necrosis. In primates, reversible renal tubular damage was observed. Clinical development was discontinued.
📄 Sci Transl Med 2011Nephrotoxicity (animal models)
In rhesus monkeys, Adipotide caused dose-dependent kidney damage (elevated creatinine). Reversible after discontinuation, but serious concern.
📄 Sci Transl Med 2011Lifestyle Factors
- Combine with a balanced, protein-rich diet.
- Aerobic and resistance exercise for metabolic adaptations.
- Adequate sleep and stress management.
- Renal function monitoring due to preclinical findings.
Injection Technique
- Clean vial and skin with alcohol.
- Pinch skin; needle at 45–90° angle into subcutaneous tissue.
- Do not aspirate; inject slowly and steadily.
- Rotate sites (abdomen, thighs, arms).
- Apply gentle pressure afterward, do not massage.
